At Bury Hospice, all our services are built around individual needs. This being the case, every patient is carefully assessed on admission and the appropriate level of care or treatment is agreed. It’s important that patients feel in control of their care, so we involve them in decision-making as much as they wish.
We also firmly believe that we are just one part of a wider caring community, so all continuing care is co-ordinated with patients, their families and carers, and other professionals. Discharge from the hospice is also determined by individual needs, so although a discharge plan will also be drawn up, this will always remain appropriate to each patient’s requirements and needs.
Discharge planning meetings maybe arranged where there are a number of significant needs to consider to ensure a safe discharge.
